Submandibular Gland Reduction

The Neck Lift That Lasts

Fullness under the jaw has everything to do with your anatomy. Tightening the skin and platysma muscles is not always enough to give you the ideal neck and jawline you envisioned. Dr. O’Daniel in Louisville, KY has more than 30 years of experience in face and neck lifting techniques. He has pioneered many of the techniques surgeons use today to create an elegant, lasting neck lift. Fullness under the jaw can come from a few different places: skin laxity, fat, muscle position, or the gland. They often overlap, and a lift alone will only ever address one or two of your concerns.

What’s Actually Under Your Jaw

Your submandibular glands sit just under each side of your jawline, and in a lot of people, they’re simply positioned in a way that shows. No amount of dieting or exercise changes that. Liposuction won’t touch it either, and neither will a standard facelift or neck lift. If your surgeon doesn’t address the gland itself with a deep neck lift, the fullness stays, no matter how well the skin is lifted and the platysma is tightened.

Dr. O’Daniel began reducing submandibular glands during deep neck lifts nearly 25 years ago. He developed a 7 step method to teach surgeons around the world how to safely reduce the submandibular glands.

The Approach: The Carina Technique

Dr. O’Daniel has developed and refined a seven -step method for reducing the glands safely, one that has since been published and taught to other surgeons.A small incision is made underneath the chin, hidden within a natural crease. From there, each submandibular gland is carefully approached and reduced using a technique specifically developed to protect the surrounding nerves and vessels while removing the volume that’s actually causing the problem.

Why Training Matters Here

A Formula One operation needs a Formula One surgeon.A procedure this precise requires a surgeon who understands the anatomy and has gone through rigorous training. Safe execution requires deep dives into advanced anatomy courses, cadaver labs, and hands-on guidance from industry experts to ensure pristine safety. The nerves running through this area control lip and chin movement. That’s not a risk Dr. O’Danieltakes lightly, and it’s not one any patient should accept from someone who hasn’t done the reps.

Throughout Dr. O’Daniel’s career, he has taught hundreds of courses to thousands of surgeons around the world and published roughly 4 peer-reviewed papers on this topic alone.

Most Recent Teachings

Dr. O’Daniel has had the pleasure of trainingmore than 400 surgeons across 65 countries through the ISAPS Face Masters program, plus hands-oncadaver courses where the anatomy iswalked through step by step. The surgeon performing this procedureshould have done this procedure often enough, under enough supervision, that it’s second nature.

Dr. O’Daniel’s Philosophy of the Elegant Neck

Dr. O’Daniel’s philosophy is “no footprints in the snow.” The goal is to make it look as if you never operated on. Soft jawlines, natural contours, nothing pulled or hollow. Some surgeons chase a more dramatic before-and-after. Dr. O’Daniel chases the version where no one can quite tell what has changed; they just notice you look rested and yourself again.

Is This Right for You?

Fullness under the jaw comes from a few different places: skin laxity, fat, muscle position, or the gland. They often overlap, and telling them apart takes an exam by an experienced surgeon, such as Triple Board Certified Facial Plastic Surgeon, Dr. T. Gerald O’Daniel.

Revision Neck Lifts

If you’ve already had a lift elsewhere and the fullness came back, or never really left, that’s one of the most common reasons patients end up finding Dr. O’Daniel. Nearly two-thirds of Dr. O’Daniel’s patients are revisional cases to correct failed surgeries performed elsewhere and restore confidence again.

Before & After

Frequently Asked Questions

What is submandibular gland reduction?

It’s a procedure within a deep neck lift that reduces the size or repositions the submandibular glands, the two glands under your jaw that can cause a persistent fullness in the neck regardless of your weight or fitness level.

How is this different from a regular neck lift?

A standard neck lift addresses skin and sometimes fat. It doesn’t address the submandibular glands. If the gland is the cause of your fullness, a lift alone won’t resolve it, and you may be disappointed with the result even though the surgery itself went fine.

Is gland reduction safe?

Done carefully, yes, though it’s a more technically demanding step than most neck surgeries. The area sits close to nerves that control lip and chin movement, so the surgeon’s experience with this specific technique matters more here than in most cosmetic procedures. Dr. O’Danieluses a seven-stepmethod, the Carina Technique, developed for safety.

Am I a candidate for this?

Possibly, if you have neck fullness that hasn’t responded to weight loss, prior neck lifting surgery, or non-surgical treatments. The only way to know for sure is an in-person exam, since fullness can come from skin, fat, muscle, or the gland, often a combination of those.

What does recovery look like?

Most patients see initial swelling settle within two to three weeks, with the final contour will be visible over the following months. Dr. O’Daniel walks through a more specific timeline during your consult based on the exact procedures you are receiving, since gland reduction is a part of the deep neck lift, and most patients are treating the face and neck with surgery.

Will my results look natural?

Dr. O’Daniel believes in subtle, natural restoration. You will still look like you. Subtly better and rested. Not obviously “done” or pulled.

Why does it matter that Dr. O’Daniel teaches this technique to other surgeons?

Dr. O’Daniel has performed this advanced procedure often enough, and precisely enough, that other surgeons want to learn it from him. Hetrains surgeons through ISAPS Face Masters and in cadaver labs. He is considered the “surgeon’s surgeon.”

What happens at my first consultation?

Dr. O’Daniel will examine your face and neck, ask about your history and past procedures, if any, and give you an honest read on whether gland reduction, a lift, fat removal, or some combination fits your case.
